LIFE is growing again and therefore is seeking 2 Full Time Case Managers (aka LIFE Coaches) to assist our wonderful residents to fulfill their independent potential by supporting them in managing their daily life skills.
Position hours are 35-40 hours primarily performed during normal business hours (M-F 8am-6pm) but may occasionally require a later evening or weekend for special events or activities. Position can also be performed with additional flexibility if needed. Payrate is $16.00 per hour and includes an excellent benefits package including Medical Insurance, Dental Insurance, Life and Long Term Disability Insurance, PTO, Paid Holidays, Sick Pay, etc.
Position would include assisting them with money management, household management, shopping, menu planning, cooking, personal presentation and hygiene, appropriate behavioral and social skills, travel arrangements, transportation needs, medical appointments, entertainment, etc.
Applicant must be patient, vibrant, energetic and highly organized with excellent time management skills; must possess excellent verbal and written communication skills; must possess excellent money management skills, strong computer skills; and the ability to multi- task. Must be a team player!
--Experience with Learning Disabled population required.
--Social Service Work experience highly desired.
--Associate's Degree, relative course work OR 3+ years of directly related experience with related population required.
*Assist assigned residents with the management of their bills and finances--overall money management
*Assist assigned residents with medical appointments and medication organization and proper documentation
*Meets individually with assigned residents, provide assessment of strengths and needs in all areas, and writes appropriate objectives and goals (ISP's)
*Assures adequate attention is given to skill and growth in all aspects of the resident's life and helps prioritize areas of emphasis in conjunction with the Employment Team
*Participates in team-related services; DDS meetings, staff meetings, housemate issues, case reviews with staff and/or family etc.
*Plans, organizes and facilitates resident activity two evenings weekly
*Role models appropriate behavior and encourages residents in leading a healthy and active lifestyle including all Covid-related precautions
*Writes service reports as routine documentation after every session or significant communication with residents
*Makes recommendations to meet needs of residents which cannot or should not be provided by the Life Coach: example: medication, therapy by licensed clinician, private tutor, house cleaner, etc.
*Focus all interactions towards helping the residents increase their independence and the quality of their day to day LIFE.
Must have current certification or be willing to attend training through LIFE for CPR/AED/First Aid and any LIFE or DDS required/recommended training classes.
Valid driver's license required with acceptable driving record and dependable transportation.
Must be able to pass CORI and fingerprinting as per DDS.
