Senator Michael Brady (Second Plymouth and Bristol district), Senator Julian Cyr (Cape and Islands district), and Senator Vinny deMacedo (Plymouth and Barnstable district) hosted a roundtable discussion on the Prevention and Wellness Trust Fund (PWTF) and funding for Community Health Centers on Tuesday, March 7 at CHC of Cape Cod. Eight Massachusetts State Senators attended in addition to various members of the Barnstable County PWTF Partnership. The group met to discuss the partnership and other community public health priorities.
The PWFT was created by the Massachusetts Legislature to help communities in their efforts to reduce health care costs by preventing chronic conditions. This broad-based coalition takes aim at diabetes, hypertension and falls among the elderly. CHC of Cape Cod was instrumental in forming this County-wide group with the following partners: Duffy Health Center, Healthy Living Cape Cod, Harbor Community Health Center-Hyannis (including Elder Services of Cape Cod and the Islands, VNA of Cape Cod and Gosnold on Cape Cod), and the YMCA of Cape Cod. Through this program, Health Center patients are referred to health, social, wellness and fitness activities provided by these partners, at no cost to the patient. Activities include Tai Chi, home safety evaluations and programs for diabetes prevention, balance and self-management.
